Work or School Note

You qualify for a work or school note if- 

- You are not being seen for workman's comp claim or by occupational health

- You are between the ages of 13 and 64

What to expect-

Our provider will review any pertinent questionnaires and may provide you with a note to return to school or work, following CDC guidelines for quarantine restrictions when appropriate.
